• Complete all sections of the Certificate in Contraception application form. This can be downloaded from Certificate in Contraception - ICGP Web Site
• Provide evidence of attendance at a theory course
• Ensure your gynaecological training is signed by your supervising gynaecologist.
• Ensure that your logbook is completed and signed by a registered Contraception Tutor. The logbook is available for download at Certificate in Contraception - ICGP Web Site
• Make payment online at the payment link provided Certificate in Contraception - ICGP Web Site
• Email a scanned copy of all documents to womenshealth@icgp.ie
