The person in this role will work closely with colleagues in the ICGP and in the Integrated Care Programme for Chronic Diseases including the national clinical programmes for respiratory disease, diabetes, cardiovascular disease as well as the NCAGL Chronic Disease in the HSE. S/he will also establish strong relationships for collaborative work with colleagues in other agencies to establish and guide the implementation of care pathways and support the programmes. This will include the implementation of the National Programme for Prevention and Management of Chronic Disease.
